1.The applicant may fill-up the request form, completely.
2. The applicant can view the fees upon filling up the google form and will be informed on the total fees to be paid through text, e-mail, or phone call.
3. The applicant deposits payment through the following Banks details account: charges and fees are based on the credentials requested for. If credentials are to be mailed, mailing fee is based on the current postal charge and the transportation if needed which will be added to the total fees to be paid.

5. The applicant uploads the copy of the deposit slip to the Finance portal. Processing of application will start upon the issuance of office receipt of payment from the Finance Office.
